Mountain Home Health Care is a Non-Profit, Home Health, Hospice, and Care-giving Agency that has been helping Taos County Families care for their elderly and disabled since 1975. We are currently seeking full-time Licensed Practical Nurses to provide case management nursing services to our patients throughout Taos County.

The ideal candidate will provide quality, skilled and/or palliative patient care to our home-bound clients, conduct home visits as assigned, assessing the patient’s condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered, while adhering to State Nurse Practice Act, agency policy, and accepted professional standards of practice, while providing nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.

Additionally, the ideal candidate will:

•Provide education according to the client's needs and within the context of the client's environment, value system, and physical/emotional status.

•Medication administrations.

•Practice nursing with respect for individual, cultural, and spiritual differences.

•Communicates effectively and in accordance with agency requirements.

•Report significant changes in client status to the physician and other members of the team in a timely manner consistent with client needs.

•Maintains client records showing systematic assessment, planning intervention, and evaluation.

•Documents legibly and according to agency documentation guidelines and standards.

•Participates in ongoing, professional self-development.

•Participates in all mandatory education per agency guidelines.

•Additional requirements of this position include:

•Current LPN licensure in NM.

•Graduate of an accredited school of professional nursing.

•Minimum of one (1) year experience in an acute care setting or equivalent experience.

•Current driver's license, auto insurance, and reliable transportation.

•CPR certification.

•Strong communication and interpersonal skills.

•Homecare/Hospice experience preferred.

Job Type: Full-time

Salary: $24.00 to $28.00 /hour
