Mennonite Village is a not-for-profit, equal opportunity employer, CCRC (Continuing Care Retirement Community) facility that provides 24 hours/7 days a week care located in Albany Oregon. We are currently hiring a Health Services Director (HSD)-RN to oversee the health care services provided for residents at Quail Run. Please note under current temporary Oregon state mandate, all healthcare staff or those working in a healthcare setting must be fully vaccinated or have a qualifying medical or religious need for an accommodation. $2000 sign on bonus. $1000 at 90 days worked and $1000 at 180 days worked ESSENTIAL FUNCTIONS: Conducts the initial resident assessment upon move-in and/or upon a change in condition. With Administrator, HSC, and Resident Services Coordinator, selects residents who have housing and health service needs appropriate within Quail Run’s Scope of Service.

Evaluates nursing staff’s ability to provide quality services for residents with increasing levels of acuity. Oversees resident transitions and move-outs.

Provides nursing expertise by regularly evaluating residents’ health-related needs and changes of health condition. Helps write interim service plans that describe current resident needs, communicates information to direct care staff and families, and guides staff in direct care delivery. As needed, communicates with physician about resident illness/health and ensures that physician orders are followed.

Provides training and guidance for medication aides. Delegates and assigns tasks based upon staff qualifications and Scope of Service. Oversees system for ordering and administering medications and treatments.

Overseas operations of FIT program.

Ensures appropriate documentation of services provided by all health services staff.

Promotes teamwork with all health services staff to provide excellent resident services.

Ensures compliance with Oregon Administrative Rules as they pertain to medical and resident care and staffing functions.

Interacts with families, pharmacists, physician office staff, and others to ensure resident needs are met with effective management of services; promotes good facility public relations during interactions.

Oversees the investigation of select resident occurrences using RL Peer software and addresses safety hazards identified within the community. Will prepare timelines and participate in the Root Cause Analysis (RCA) process for high severity events within a timely manner. Institutes correction and prevention activities and includes them in written service plans. Promotes safety practices by nursing staff. Participates in QA team to improve quality.

Address problems or concerns appropriately and promotes safety practices in nursing.

Regular attendance is an essential function of the job.

Immediately reports identified hazards or hazardous conditions to a supervisor.

Immediately reports adverse events and near misses to a supervisor.

Attends mandatory training on HIPPA and compliance annually.

QUALIFICATIONS:

Must possess current and unencumbered RN license in the state of Oregon.

Must have the ability to plan, organize, and direct care.

Must be able to work On-call and Holidays as needed.

Must demonstrate leadership, facilitate teamwork and be willing to participate in direct caregiving activities.

Must be able to relate to residents and staff in a courteous, diplomatic manner in all circumstances.

Must desire to work with people with Dementia and understand the philosophy behind the Social Model of Care.

Ability to be flexible, patient, creative.

Demonstrated ability to direct and manage personnel.

Must possess strong organizational and team-building skills.

Ability to be objective and treat staff fairly and equitably.

Must be capable of working flexible hours, occasionally weekends and holidays, and be accessible by phone to provide direction to non-nursing personnel.

Ability to work with and communicate effectively with the aging.

Demonstrated understanding of physical and emotional changes with aging.

Must possess sight and hearing senses within normal limits.

Must be able to use good body mechanics to move safely during the day including moderate lifting (25 lbs.), carrying, bending, stooping, and kneeling.

Must be able to assist with one and/or two persons transfer of residents.

Ability to prioritize workload and adjust priorities on short notice.

Capable of always maintaining confidentiality of information.

Ability to delegate certain tasks in accordance with State Board of Nursing

Capable of lifting 10-50 lbs., with occasional lifting of residents weighing over 100 lbs.

Ability to safely move frequently during workday, including carrying, lifting, standing, sitting, bending reaching, and kneeling. Ability to push and/or pull a wheelchair.

Visual and auditory acuity within normal limits

Sufficient English and Language skills to read care plans and communicate effectively with other staff. Must be able to understand and respond properly to resident’s needs.

Ability to accurately document activities in writing.
