Using standards of dietetic practice, provides optimum nutritional care to assigned patients.
Completes nutritional consults in an appropriate and timely manner
Assesses nutrition needs and determines nutrient requirements and appropriate diet according to patient's age, disease-specific needs, behavioral and metabolic response to medical and nutritional care.
Collaborates and communicates with patient and health care team to assess needs, establish mutual goals and determine the most appropriate care plan to meet goals.
Identifies desired outcomes of nutrition care, verifies that each nutrition-related problem listed on the medical record has been addressed.
Conducts mealtime visits to evaluate patient's acceptance, tolerance or rejection of nutritional regimen.
Provides patient and family nutrition education/counseling, as needed. Provides appropriate materials to facilitate learning/change.
Evaluates effectiveness of nutrition intervention/education in terms of patient's therapeutic outcome and behavioral response.
Documents in patient medical and dietetic records the nutrition care provided, including communications with medical team, results and revisions in nutritional care, patient's behavioral response and therapeutic outcome to nutrition interventions and/or education.
Informs healthcare team about regulations, professional guidelines, departmental policies and procedures that are related to nutrition care.
Attends and participates actively at weekly Patient Care Rounds and Discharge Planning meetings.
Keeps abreast of current research findings to enhance knowledge and skills. Shares knowledge with nutrition staff and health care team. Provides lectures to community, as assigned.
Communicates the need for and priorities of relief coverage to head nurse for own unit or other units in a timely manner. Ensures that all nutrition consults and diet order triggers are completed in an appropriate and timely manner.
Performs related duties, as required.
ADA Essential FunctionsJob Qualification
Bachelor's Degree in Nutrition or related field, required. Master's Degree in Nutrition or related field, preferred.
Registered Dietitian, required.
Minimum of one (1) year clinical experience in acute or long-term care facility.
Good interpersonal and communication skills required.
ICU and Med Surg experience, preferred.
