Builds rapport and demonstrates a safe environment in which patients engage in the comprehensive assessment processPerform ongoing continuous assessments to develop and maintain appropriate, safe, strengths-based and person centered care. Utilize diagnostic criteria, symptoms, differential diagnosis, and rule outs to determine accurate diagnosis Individual CounselingResponsible for an individual case load including 1:1 counseling, treatment, relapse prevention, discharge planning, care coordination and referralParticipate in case conferences and clinical meetings weekly. Provide individualized patient care utilizing best practice counseling skills, techniques and modalities to improve quality of life and reduce symptomsUtilize crisis intervention skills and community resources to promote patient safetyUtilize trauma-informed approaches including taking universal precautions Engage in ongoing professional maintenance activities to include but not limited to clinical supervision. continuing education, and counselor wellnessGroup/Family Counseling and EducationDevelopment, execution and documentation of group therapy on assigned topics to be determined by patient need and counselor skill setAwareness of group dynamics, ability to manage time and destructive behaviors, encourage participation and engage patients in the learning process. Conduct family, marriage and or couples therapy as clinically indicatedProvide psychoeducation to individuals, groups and families on topics such as substance use disorders, mental health and wellness, life style changes, and coping skills, and recovery. Strengthen relationships among people in a purposeful effort to promote, restore, maintain, and enhance the well-being of individuals, families, social groups, organizations, and communities.DocumentationDocumenting: In a variety of manners including assessments, individual and group notes, collaboration, collateral contact with internal and external stakeholders to the patient. Treatment planning, discharge planning and regularly updating the patient record to demonstrate patient's progress or lack thereof in the treatment environment. Assessments are performed per specific scope and practice at or above standards set by OASAS, OMH, TJC and DNV.Forms are filled out respecting the various deadlines required by the governing and regulatory bodies and will remain in the medical record.Confidential maintenance of and transmission of medical records following HIPPA and CFR 42 guidelinesClinical SupervisionProvide clinical supervision for interns and or junior staff of the same disciplineAct as a mentor, coach and evaluator to create an atmosphere that promotes counselor self- reflection, discipline, motivation, learning, wellness and professional development. Monitor quality of professional services, attend to ethical issues and ensure quality clinical care. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.PAY RANGE: $58,240.00 - $76,336.00The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
