Comfort Care Home Manager

We are seeking a dedicated and experienced Home Manager to oversee the daily operations of our end-of-life care home. The ideal candidate will have a strong background in hospice, and operations management. As the Home Manager, you will be responsible for ensuring that residents receive compassionate care while maintaining a safe and comfortable environment. You will lead a team of nurturing volunteer caregivers and support staff, develop care plans, and ensure compliance with all regulatory requirements.

Responsibilities

Oversee daily operations of the home, ensuring adherence to policies and procedures.

Supervise staff, providing guidance and support to ensure high standards of care.

Develop and implement individualized care plans for residents in collaboration with healthcare professionals.

Monitor resident health and well-being, addressing any concerns promptly.

Ensure compliance with state regulations and requirements for Comfort Care Home facilities.

Oversee medication administration processes to ensure safety and accuracy.

Facilitate communication between residents, families, and staff to promote a positive living and working environment.

Maintain and foster professional relationships with City of Kingston officials, Hudson Valley Hospice, hospitals, medical providers, religious organizations, and the human services community at large.

Conduct regular assessments of resident needs and adjust care plans as necessary.

Organize training sessions for staff on best practices in caregiving and safety protocols.

Collaborate with the CFD Board and Financial Committee with creating and managing annual operating budget, and assist with occasional fundraising activities.

Qualifications

Proven experience in operations management within a palliative care, hospice, or care home setting. Strong familiarity with the Comfort Care Home model is ideal.

RN, BSN, or BSN in Health Care, Administration, and or Palliative/Hospice care.

Strong supervisory skills with the ability to lead a diverse team effectively.

Knowledge of care plans, medication administration, and regulatory compliance in a hospice facility.

Experience in social work or related fields is advantageous.

Excellent communication skills, both verbal and written, with a compassionate approach to resident care.
