Looking for a reliable Private Duty LPN or RN nurse to care for adult male patient with muscular dystrophy in his home. We are currently looking to fill Wednesday & Thursday, 9:00am to 9:00pm with possible fill ins on weekends or other days. He is wheelchair bound, has a feeding tube, catheter, and trach. We have an electric hoyer lift for transfers. We are located in Mahopac, NY (Putnam county) & prefer someone local to the area. You must have a medicaid provider number and be able to bill Medicaid on your own for a high rate patient.
