Overview MHA in Ulster County, Inc. has a Full-Time (35 hours per week) position as a Supported Education Coordinator and Transitions Coordinator. This position has a dual caseload with half the time working as a Supported Education Coordinator and half the time as a Transitions Coordinator. The Supported Education Coordinator assists with the implementation of all aspects of the ACES supported education program. This position provides individualized assistance to all students in the program with admissions and financial aid applications. Throughout the semester, this position helps students manage the college environment successfully. The Transitions Coordinator coordinates and provides advocacy for young adults, between the ages of ages 18 and 26, that are homeless or at risk of being homeless and have drug and alcohol / mental health issues. The goal of the program is to help transition-age youth function more independently and successfully across life areas to increase independence and promote recovery. The schedule for this position is Mon-Fri 9am - 4:30pm. This position makes a pay rate of $20.66 hourly. Responsibilities and Duties Supported Education Coordinator
Process authorizations and intakes of applicants referred to the ACES program.
Assist students individually with the completion of necessary forms (i.e., admissions, financial aid) and with negotiating various college systems.
Develop supportive relationships with students and maintain ongoing, continuous contact with them for as long as they are attending college.
Work closely with MHA’s Finance Department to track ACCES-VR billing.
Transitions Coordinator
Provide or coordinate services such as information, referral, emergency housing, clothing, transportation, legal and entitlement advocacy, peer support, vocational, life skills training and education, and crisis intervention when needed.
Manage a caseload of 18 clients and develop a Service Plan with each individual based on prioritized goals.
Identify and develop a safety plan to address any imminent safety concerns.
Provide or coordinate consumer education and staff training on mental health and substance abuse issues and other needs as they arise.
Initiate and facilitate communication with community agencies in order to refer and link individuals to resources/ supports to meet service plan goals.
Provide a minimum of bi-weekly individualface-to-face visits with the individual to ensure continuous monitoring of progress and/or revising of Service Plan
Qualifications and Skills
A Bachelor's Degree in a Human Services Field and 2 years’ relevant experience
Familiarity and comfort with the postsecondary setting, college forms, procedures and systems
Must have a clean, valid NYS driver’s license with 6 years driving experience.
Ability to safely perform the essential functions of the position, with or without accommodations required by federal, state, or local law.
