POSITION SUMMARY
The Floor Assistance / Program Aide provides direct supervision, physical assistance, support with ADLs, cleaning, and safety monitoring for all program participants.
This position ensures a safe, clean, and supportive environment in accordance with DOH and HCBS standards.
DUTIES & RESPONSIBILITIES
1. Supervision & Safety
Maintain continuous supervision (line of sight) of all participants.
Prevent falls, accidents, and unauthorized exits
Monitor participants for physical, emotional, or behavioral changes.
Assist with wheelchairs, walkers, and mobility equipment.
2. Physical Assistance – Lifting / Transfers
Ability to lift, move, or assist up to 50 pounds safely.
Perform safe transfers including:
wheelchair ↔ chair
chair ↔ restroom
assisting with ambulation
Follow proper body mechanics to prevent injury.
Assistance with ADLs (Personal Care)
Assist participants with restroom use, including:
guiding in and out of the restroom
hygiene support
continence care and diaper changes if needed
Assist with handwashing, personal hygiene, and mobility.
Assist during meals: serving food, cutting food, opening containers.
4. Cleaning & Basic Housekeeping
Take out trash from the assigned program area.
Clean and disinfect:
tables
chairs
activity areas
restrooms (as assigned)
frequently touched surfaces
Maintain a clean, organized, and hazard-free environment.
Provide light support to the kitchen as needed.
5. Program Activities
Assist in setting up and supporting recreational, cognitive, and social activities.
Encourage participant engagement and maintain structured activity groups.
Accompany participants during in-center and community activities (if assigned).
6. Documentation & Reporting
Complete attendance sheets, incident reports, ADL notes, and behavior documentation.
Immediately report any accidents, unusual behaviors, or emergencies to management.
7. Communication & Teamwork
Communicate professionally with participants, families, and staff.
Collaborate with Activities, Case Manager, Kitchen, Transportation, and Front Desk teams.
Attend required trainings and staff meetings.
