Private Duty nurse (Medicaid Private Provider) needed for a home care case in Coram (Suffolk County) for two kids with Cerebral Palsy. This is a shared case: a set of twins who are non-ambulatory, eat foods by mouth and require full assistance with ADLS.
The shifts needed are Monday-Thursday 6:30am-8:30am
The nurse must have their NPI # and be able to bill Medicaid for compensation.
