Small NFP is seeking a Social Worker to work with our patient service program serving the ALS community on Long Island. The candidate will work a flexible 4 day schedule/location in our Stony Brook University office and some field visits. Responsibilities include administering patient programs, assisting families, tracking patients and in-home-visits. Hours average 15-25 per week.
