Per Diem
Hours: 21 hours per week
Location: Fully Remote
Start: ASAP
✅ Ideal Candidate:
Registered Nurse or Nurse Practitioner
3–5 years of experience in care management, hospice, or palliative care
Strong knowledge of:
Advanced Care Planning
Goals of Care discussions
End-of-life care principles
Associate's degree in nursing required
Bachelor’s Degree preferred
CHPN Certification is a plus
Salary RN - up to 63/hr
NP - up to 75k
