Concern for Independent Living, Inc. is a non-profit agency committed to helping individuals and families to live in the community with dignity and enhanced opportunities through the provision of housing and support services. We are one of the largest housing agencies of this kind in New York State, currently serving approximately 1,300 individuals and families in over 240 locations. Concern offers a variety of housing options with individualized support services designed to support personal growth and independence.
The Counselor is responsible for assisting the Supervisor and Case Managers in the delivery of direct services to the clients, as well assisting in the overall running of the site. The counselor provides a variety of services, ranging from the teaching of skills to delivery of concrete services to providing counseling to the clients.
Duties and Responsibilities:
â¾Provides direct care services to a caseload of clients.
â¾Responsible for documentation of clients assigned to as well as documentation relating to other clients progress.
â¾Monitors the site, coordinates and participates in house activities such as preparation of meals, chore activities, recreational activities, safety checks etc.
â¾Facilitate food-planning committee; train clients in weekly food planning.
â¾Provide rehabilitative support services in daily living skills, health services and others as assigned.
â¾Provide housing services including assistance with maintenance issues.
â¾Monitors medication.
â¾Assists with transportation of clients.
â¾Attend staff meetings as scheduled.
â¾Perform monthly fire inspections.
â¾Remain current with all Agency required compliance documents and trainings.
Qualifications: Part-time positions require experience working a similar setting or with people with mental illnesses.
Schedule: Saturday & Sunday; 8am - 4pm
