Registered Nurses – Home Care (Queens, NY)
Pay Rate: $68–$70/hour
We are seeking experienced and compassionate Registered Nurses to perform in-home clinical visits across various neighborhoods in Queens, NY.
Hiring in the following areas:
Forest Hills
Glendale
Rego Park
Ridgewood
Jamaica
Northeast Queens
Responsibilities:
Practice independently in the community as part of an interdisciplinary care team
Deliver personalized nursing and care management services to patients in their homes or care facilities
Conduct Start of Care (SOC) and Resumption of Care (ROC) visits
Complete detailed OASIS documentation and CHHA assessments
Evaluate evolving patient needs and proactively adjust the care plan
Qualifications
Current license to practice as a Registered Nurse in New York State
Minimum of one year of nursing homecare experience
Proficiency with OASIS documentation
Reliable transportation required
Valid driver’s license or NYS Non-Driver photo ID card may be required
