We are looking for a Staffing/Case Coordinator, who's a Bilingual (English/Spanish Speaking) Full-Time Job For a Home Care Agency in (Brooklyn)

Employment type: full-time

Job title: Home Care Staffing/Case Coordinator

A full-time case coordinator is needed to work with this home care agency and can start ASAP.

Office Hours 8 AM-5 PM

Bilingual English/Spanish.

What we're looking for:

Previous experience in scheduling or coordination, preferably in a healthcare or home care setting

Excellent communication and interpersonal skills, enabling you to build rapport with clients, caregivers, and healthcare professionals. ยท

Ability to adapt to new software and work models.

Knowledge of HHAeXchange

Must be good with using technology: electronic systems, emails, word, PDF, etc.

Experience preferred, but not required.

Job Duties:

Schedules case coverage, always informing both patient and/or significant other of the name of the scheduled caregiver.

Sending vendor communications to inform insurance providers about PTs.

Verification of aides' attendance, Pre-Billing, linking calls, and entering timesheets as necessary for payroll and program purposes.

Updating Schedule/ Master week and entering appropriate notes in the HHA exchange system.

Participates in basic case management activities.

Fill out appropriate reports and send them to the Supervising Nurse if Falls, Hospitalization, /incidents are Reported.

Participates in QA meetings.

Participates in agency On-Call.

Answers telephones and responds to inquiries by directing calls to appropriate staff if necessary.

Other assignments as directed to facilitate daily operations.
