I am pharmacy student and I need to conduct an interprofessional zoom interview relating to a fictional parkinsons patient:

Looking for Nurse, NP, PA, Physical Therapist, Dietician, clinical social worker . Interview should take no more than 30 minutes max

Case:

Name: Teresa Marks Gender: Female Age: 76 Race: Caucasian

Allergies: NKMA

PMH: Hypertension, GERD (gastroesophageal reflux disease), Parkinson’s disease

Social history: Married x 53 years. Non-smoker. Negative for alcohol consumption. Lives at home with

her husband.

Medication: carbidopa/levodopa 25/250mg po 5x/day, pramipexole 1.5mg po tid, lisinopril 5mg po

daily, pantoprazole 40mg po daily.

Ms. Marks was diagnosed with Parkinson’s disease 13 years ago. She has a visible tremor in her upper

extremities, with more prominent symptoms on the right side. She walks on the balls of her feet, and is

unsteady without a walker. Over the past several months, she has fallen 3 times, and required stitches

in her forehead as a result. Recently, her husband states that he has noticed Ms. Mark’s short term

memory has become quite impaired. This is confirmed by the fact that the patient’s score on the mini

mental state exam is 17, which is indicative of moderate dementia. She also mentions that she

frequently sees their dog Angus wandering around the house recently despite the fact that he actually

passed away 7 years ago. Mr. Marks is concerned that Teresa is losing weight, and is having trouble

swallowing food and her medications without choking. She has very little appetite, and is having

increasing difficulty using eating utensils due to her tremor. She also needs help dressing herself in the

morning due to decreased fine motor control. Ms. Marks complains of cramping in her lower

extremities, especially her feet, which causes her to wake up during the night. This in turn is adding to

her daytime drowsiness. Mr. Marks is aware that having his wife living with him at home may be

detrimental to her safety, and that it may be time to seek other living arrangements. He is struggling

emotionally with the situation, and seems to be depressed.

We will be on zoom camera. Must available for a 30 min slot between 5pm- 10pm tonight(Monday) and from 9am-5pm tommrow(Tuesday) .Must be liscensed in any state. Must agree to being on camera(can use photo, but audio must be on) and being recorded. zoom session will be recorded. Must be able to provide ID and sign a contract for payment.
