Advance Care Alliance (NYC & Long Island) Care Manager ($40K Salary) (Must have BA/BS with 2-years of relevant experience working directly with people with physical and/or intellectual challenges, or a license as a Registered Nurse with two years of relevant experience, or a Master's degree with one year of relevant experience.)
Advance Care Alliance (NYC & Long Island) Care Manager Supervisor ($50-60k Salary) (1-year experience providing Care Management (MSC) + Master's degree or 2 years' experience providing Care Management /(MSC) + BA)
Advance Care Alliance (NYC & Long Island) Director of Care Management ( Over $60k Salary) (2 years supervising MSC/Care Management team. BA + 5 years' experience in I/DD Care Management or master's degree + 3 years' experience in I/DD Care Management.)
Advance Care Alliance (NYC & Long Island) Bilingual Intake and Benefit Access Coordinator ($45k Salary) (Bachelor's degree plus one-year experience completing enrollment for OPWDD supports and services. REQUIRED to be proficient in Medicaid insurance (experience in other insurances are not valid for this position). Must be able to speak Mandarin, or Cantonese.)
