Responsibilities:
Provides case management to individuals to help them understand and overcome personal or social issues.
Coordinate counseling and therapeutic services with other professionals such as doctors, nurses, psychiatrists, and social workers.
Conduct consistent follow-up interviews with clients to determine if their needs are met.
Maintain appropriate and timely documentation, progress notes, and screening/assessment tools.
Responsible for caseload updates, data collection, and submission of activities reports.
Ensure that clients are housing ready.
Prepare housing packages
Ensure that clients are keeping all outside appointments.
Qualifications:
Minimum associate degree
Multi-culturally competent and strong interpersonal skills
Knowledgeable of all city and state & federal entitlements benefits
Knowledgeable of various community resources and partners
