Full-Time Case Coordinator Needed for Home Care Agency in Rego Park to start ASAP.
Office Hours 9AM-5PM
Bilingual English/Spanish or English/Russian.
Must be good with using technology: electronic system, emails, word, pdf, etc.
Experience preferred, but not required.
Job Duties:
Schedules case coverage, always informing both patient and/or significant other of the name of the scheduled caregiver.
Sending vendor communications to inform insurance providers about PT's.
Verification of aides' attendance, Pre-Billing, linking calls, entering timesheets as necessary for payroll and program purposes.
Updating Schedule/ Master week and entering appropriate notes in HHA exchange system.
Participates in basic case management activities.
Fill out appropriate reports and sending to Supervising Nurse if Falls, Hospitalization, /incidents Reported.
Participates in QA meetings.
Participates in agency On-Call
Must be available to do at least once a month on call to answer calls on weekend and after hours!
Answers telephones and responds to inquiries by directing calls to appropriate staff if necessary.
Other assignments as directed to facilitate daily operations.
