Nurse (LPN/RN), Private duty (PDN), in home, Medicaid # req (Penfield)
Private-duty nurse (LPN/RN, must have NYS Medicaid number) for 21-year old female in our home.
We are looking for a nurse with availability for periodic fill-in and some scheduled shifts per month. This may change over time but our current need is not for a specific day of the week or shift. See below for more information.
PATIENT
Our 21-year-old daughter has Pura Syndrome, a rare genetic disorder caused by mutations in the PURA gene. As a result she is extremely delayed both mentally and physically and has seizures which are under control through medications. She is low-tech, g-tube fed, non-verbal, non-ambulatory. Despite her disabilities she is a very pleasant young lady who is always in a good mood and excited to engage and interact.
DUTIES
Duties include preparing and monitoring g-tube feedings, administering chest vibration vest, preparing and administering medications, giving daily baths (we have a track and lift). She weighs approximately 100 lbs. Some lifting/transfers can be performed although the track and lift can be used for all transfers if desired.
NURSING/SHIFT HOURS
Our daughter attends a 12-month adult program (day-hab) and we have in-home nursing during periods when she is home and not at program.
Nursing hours are normally weekday evenings (4/5pm -8/9pm) and weekend day/event (two shifts, normally noon-5pm and 5pm-10pm).
In addition we have some daytime hours when program is off (holidays) and also periodic 24-hour in-home respite.
We run a flexible month-to-month based schedule. We have multiple nurses to fill the shifts and allow most to vary their shifts each month based on their availability that month.
As stated above we'd like to find someone who is interested in some scheduled shifts per month (maybe averaging one or two per week depending on your availability) and periodic fill-in opportunities. Some of our nurses work a full-time job at a hospital or facility and then some home care with our daughter, while others only work home care (ours and others).
The work environment is clean, friendly, and this is a great opportunity for the right person.
