Some flexibility is required based on the needs of the agency and the individuals we support. Full-time, 37.5 scheduled hours weekly. POSITION SUMMARY:Responsible for managing the day-to-day responsibilities of administering habilitation/community services to individuals with developmental disabilities in St. Lawrence and Franklin Counties in compliance with service plans, agency policy, OPWDD regulations, QA, and consumer choice.PRIMARY RESPONSIBILITIES:

Develop, coordinate, and supervise the implementation of each person’s habilitation plan to improve self-sufficiency through increasing independence, productivity, and inclusion in the community and participation in appropriate social, educational, and recreational activities.

Develop each person’s safeguards and keep updated with changes in Precision Care. Provide training to staff on each person’s needs and plans as written.

Maintain petty cash, balance accounts, and complete related documentation.

Oversee the use of agency vehicles, including coordinating the use of agency vehicles for program participation, completing vehicle logs, submitting gas receipts, and ensuring agency vehicles are in good repair by submitting work orders for repairs to the Facilities Department as necessary.

Interview, train, and supervise direct care staff, including creating caseloads for each staff member, training staff on creating work schedules to meet the needs of people receiving services, preparing evaluations, overseeing time records, commendations, and disciplinary guidance.

Coordinate/communicate with the Training and Development Manager to ensure staff are completing required training.

Ensure each person receives services as prescribed in their Life Plans, Staff Action Plans, and Individual Plans of Protective Oversight.

Create and implement protocols to be followed for concerns and/or emergency situations for staff working outside regular business hours. (i.e. after 4 and weekends)

Report all potential instances of abuse, neglect, and/or sensitive situations to the supervisor/administration and NYS Justice Center as required by regulation.

Ensure compliance with all regulatory requirements for continued funding/certification of OPWDD programs.

Ensure staff completes the required documentation, including daily habilitation notes and monthly progress notes.

Ensure billing is completed within the appropriate timeframe and all billing is accurate. Review all notes in Precision Care and correct errors through communication with the writer. Complete Billing Change forms as necessary.

Report units billed to Fiscal Intermediaries for individuals with Self-Direction services monthly or as FI requests.

Communicate frequently with people receiving services, families, advocates, service providers, and other members of the planning team.

Provide direct care service to people receiving services as necessary to meet program needs and/or to provide coverage for staff on leave.

Perform other duties as requested by supervisor.

Requirements

A Bachelor’s Degree in human services or a related field and three years of experience in the DD field arerequired.

QIDP statuspreferred.

Two years of supervisory experience isrequired.

Effective verbal and written communication skills.

Strong organizational and computer skills, including Microsoft Office Products.`

Ability to maintain good working relationships with others.

Exhibits a high level of professionalism and confidentiality when dealing with issues.

Valid NYS Driver’s License and personal vehicle for business use.

Acceptable motor vehicle abstract to meet agency insurability standards.

Ability to lift and carry up to 25 lbs. and lift/transfer and support consumers with and without help.

Ability to stand/walk for long periods of time.

Some exposure to infectious diseases, hygiene issues, food, and other odors.

Able to tolerate atmospheric conditions when performing regular outdoor seasonal activities.

Ability to operate mechanical devices used by consumers.

Ability to tolerate extensive exposure to computer monitor/screen.

Ability to tolerate extensive use of computer keyboard.
