Evaluate and Develop Treatment Plans: Assess residents based on their medical diagnoses and create individualized treatment plans to enhance functional independence.
Implement and Monitor Progress: Deliver therapeutic interventions that support residents in achieving their personal goals, while closely tracking progress in recertifications, progress reports, and daily notes.
Confidentiality and Documentation: Ensure the privacy of resident information by adhering to confidentiality protocols and maintaining accurate documentation of care services.
Clinical Supervision: Oversee and clinically supervise occupational therapy assistants, ensuring quality care is provided.
Collaborative Care Planning: Participate in care plan meetings, contributing to comprehensive documentation and team discussions.
Data and Report Management: Plan, organize, and develop OT-related reports and assist with data tracking as directed by the Rehabilitation Director.
Support Departmental Initiatives: Assist with various departmental projects and tasks to support team objectives.
Requirements:
Graduate of an accredited Occupational Therapy Program
Current New York State Occupational Therapy License
Prior experience in long-term care is preferred, but not required
