Req Number 138046Job DescriptionLeads and guides MDS coordinators in the organization of assessment for patients/residents to comply with Centers for Medicare and Medicaid Services (CMS) and Department of Health requirements; assessing residents for Medicare and managed care benefits; ensuring continuous maintenance of Minimum Data Set (MDS) computer-records. Collaborates with management to create the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care.Job Responsibility

Prioritizes schedules and assigns work to optimize operational service.

Provides staff support including assistance with problem solving and recommendations for process improvement.

Participates in performance improvement processes both within the facility and with transferring facilities to maximize reimbursement.

Carries out routine correspondence.

Ensures compliance to organization policies and procedures, and regulatory requirements.

Oversees compliance with government and agency regulations.

Uses knowledge and skills to contribute to the development of functional objectives and principles to achieve goals in innovative ways. Problems addressed are of diverse scope and complexity ranging from moderate to substantial.

Job Qualification

Graduate from an accredited School of Nursing required.

Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications (RAC) as needed.

5-7 years of relevant experience and progressive experience where team leadership was demonstrated, required.
