We stand out by providing personalized attention and support within the comfort of our patients' homes, making a lasting difference in their lives. Responsibilities: Clinical Training and Education: Provide instruction and training on clinical skills and procedures during the initial hiring process and continuously as needed, with an emphasis on the professional development of field staff. Supervision of Home Care Personnel: Supervise all home care personnel, including non-licensed staff, LPNs, and RNs, ensuring appropriate task delegation, compliance with state guidelines, and adherence to best practices. Coordination of Patient Care: Facilitate effective communication to ensure the seamless implementation of each patient’s Plan of Care. Support and Guidance: Offer support, guidance, and education to patients, their families, and caregivers to promote optimal physical and emotional well-being. Mandatory Reporting: Identify and report any observed or suspected patient abuse, neglect, or exploitation to the appropriate authorities as required by law for health professionals.
