Description

1.Assists with compliance / knowledge of the cancer program

2.Helps with physician and patient follow up as directed

3.Assists with patient navigation services

4.Gathers and assesses information regarding the patient’s physical needs, mental status, family support system, financial resources and available community and governmental resources.

Uses assessment to develop a comprehensive case management plan that will address identified needs.

5.Provides information about resources and options available in the community and coordinates service delivery by making referrals to community resources in collaboration with physicians and patient choice.

6.Provides counseling services in individual, group and family settings as needed.

7.Provides factual information based on current knowledge to provide psychosocial support and assist the patient / family in coping with their disease to improve their overall health care management.

8.Monitors patient care across the continuum through follow-up with patients, families and community services.

9.Coordinates and initiates training / orientation programs for staff, coordinates ongoing education presentations.

10.Maintains communication with patient navigator, physician’s and health care team members as identified to provide information regarding case management plan.

11.Participates in performance improvement activities specific to departmental functions, system wide initiatives and regulatory guidelines.

Assists with cancer specific accreditations.

12.Assists with program development and outcome tracking (ie. smoking cessation counseling).

13.Collaborate with service line manager to determine fiscal requirements of program and prepare budgetary recommendations.

14.Participates in cancer care conferences, and cancer committee

15.Represents patient / family by intervening, negotiating and promoting their concerns. Problems requiring advocacy may include individual and class inequities or inadequate and non-existent hospital and or community resources (insurance benefits, housing etc.)

16.Maintains pertinent and timely documentation in patient medical charts and departmental records.

17.Participates in community outreach activities

18.Performs other related duties as required.

19.Adherence to all core measure initiatives, patient safety goals, and NQF Safe Practices

Qualifications

MS in Social Work, Psychology, Sociology or other related field.

Certified Social Worker from NC Certification Board for Social Work

Licensed Clinical Social Worker Certification

MINIMUM EXPERIENCE

Experience and knowledge of social workprinciples, techniques, and practice required. Previous experience withcounseling and billing practices required.

Must have excellent communicationskills, demonstrate self-direction, and have the ability to work well in a teamenvironment.

Leadership or management experiencepreferred

OTHER SPECIAL KNOWLEDGE OR SKILLS

Knowledge of oncology disease andtreatment process and effect of illness on patient population

Knowledge of resources specific tooncology

Ability to work independently with establishedguidelines

Ability to relate to peoplelike in a positive manner regardless of personal preference or beliefs.
