Position Summary:
We are seeking a compassionate, reliable, and proactive Day Support Worker to provide individualized support to an adult female with Autism, developmental disabilities, mental health diagnoses, and Type 1 Diabetes. The ideal candidate will foster independence, inclusion, and engagement by promoting community participation, supporting life skills development, and ensuring the individual’s overall well-being in a safe and respectful environment.
Key Responsibilities:
Individualized Support: Provide one-on-one assistance tailored to the individual's goals, needs, and preferences.
Community Engagement: Facilitate outings into the community, including recreational activities, volunteering, social events, and participation in local programs to promote inclusion and engagement.
Life Skills Development: Teach and support daily living skills such as cooking, budgeting, time management, hygiene, and public transportation use.
Health Monitoring: Support the management of Type 1 Diabetes, including blood glucose monitoring, insulin administration, recognizing symptoms of hypoglycemia/hyperglycemia, and responding appropriately in emergency situations.
Emotional and Behavioral Support: Implement positive behavior strategies and support emotional well-being in line with mental health best practices and the individual's care plan.
Health and Safety: Monitor and support the individual's physical and emotional health, administer medications as required, and follow all safety and care protocols.
Documentation: Maintain accurate records of daily activities, progress toward goals, health data (including diabetes management logs), incidents, and other relevant information in compliance with agency policies.
Collaboration: Work closely with family members, healthcare professionals, therapists, and support teams to ensure a consistent and holistic approach to care.
Transportation: Provide transportation to and from appointments, activities, and community outings as needed (valid driver’s license required).
Qualifications:
High school diploma or GED required; post-secondary education in Social Services, Developmental Services, or a related field is preferred.
Minimum of 1–2 years of experience supporting individuals with developmental disabilities, Autism, and/or mental health conditions.
Knowledge and/or experience in diabetes (particularly Type 1 Diabetes) is strongly preferred; training can be provided.
Familiarity with person-centered practices and the implementation of individualized support plans.
Strong interpersonal and communication skills.
Patience, empathy, and a genuine commitment to enhancing quality of life.
Valid driver’s license, reliable vehicle, and clean driving record.
Current First Aid/CPR certification (or willingness to obtain)
Criminal background check required prior to employment.
Work Environment:
Community-based with some in-home support as needed.
Physical activity required, including walking, standing.
Flexible part-time schedule (approx. 20 hours/week), with occasional evenings or weekends based on the individual’s needs.
