Description/Job SummaryThe mission of Resources for Human Development (RHD) is to provide caring, effective and innovative services that empower people of all abilities as they build better lives for themselves, their families and their communities. RHD's multi-faceted programs serve people with a variety of challenges that include intellectual and developmental disabilities, mental illness, substance abuse and homelessness.The LTRP Case Manager will work with a team of staff and individuals receiving service to ensure that their health, safety, skill building, community integration and coordination of service needs are being met. The LTRP program emphasizes on assessing indvidiuals, supporting skill building and linkage to community resources. The Case Manager reports to the Program Manager.Schedule Available:

Saturday-Sunday: 7am-3pm & 3pm-11pm

The Primary location for this program is Durham, NC.Responsibilities/Duties

Complete Treatment Plan, Assessments and other required documentation with individuals via EHR

Engage in Person Centered planning and goal reviewing

Advocate for consumers' rights and preferences

Consult with community agencies, guardians, and families to maintain coordination in the treatment process

Monitoring for health & safety of consumers

Linking to outside resources

Assisting with maintaining benefits

Provide ongoing assessment, problem solving, side-by-side services, skill training, supervision (e.g. prompts, assignments, monitoring, and encouragement) and environmental adaptations to assist consumers with activities of daily living.

Assist and support consumers to carry out personal hygiene and grooming tasks.

Provide nutrition education and assistance with meal planning, grocery shopping and food preparation.

Assist and support consumers by teaching skills necessary to perform household activities, including house cleaning and laundry.

Provide side-by-side support and coaching to help consumers socialize.

Assist consumers with medication monitoring and management (e.g. ensure that clients are taking medications as prescribed by treating physician).

Required ExperienceMinimum of 2 years' experience working in a residential facility or mental health setting is preferredRequired EducationBachelor's degree in Human Services field i.e Social Work, Psychology, Criminal Justice etc.Required Qualifications

Valid Driver's License

Must Pass Criminal Background Check

Program SummaryRHD provides a variety of services tailored to meet individual needs and person-centered goals. The focus of the LTRP is on continued recovery and further developing life skills that are essential to successfully living more independently in the community.
