Operation Homes Case Manager

The Case Manager position covers a variety of facets within the Operation HOMES organization and exists to support the program guests as well as the host site churches. Depending on the number of guests and applications, this position requires 5-15 hours per week The duties of the case manager include:

Meeting with current guests weekly to assess their progress and make their case plan for the week.

Communicating with Medina Metropolitan Housing Authority to receive applications for guests as well as help current guests with housing/voucher applications.

Interview potential guests and determine if they should enter Operation HOMEs, refer them elsewhere if Operation HOMES is not an option for them.

Updating the host sites weekly on the progress of the guests.

Working directly with each host site coordinator during the week that the guests stay with them to help with any guest issues, coordination with the next church, etc.

Respond to calls on the Operation HOMES phone.

Assisting guests with housing and job searches, finding transportation, applying for food stamps, phones, and medical, building budgets etc. Help guests find the tools they need to be successfully housed and able to live on their own.

Attend the monthly board meetings every third Monday at 7:00 PM.

Communicating and meeting with the Consultation committee regarding guest intake, case plans, and behavior.

Arrange for transit for guests without cars to get to and from the host site with Medina Public Transit.
