We are seeking a compassionate, skilled Physical Therapist (PT) to join our clients' Home Health team. In this role, you will provide comprehensive in-home physical therapy services, complete evaluations, and develop individualized treatment plans to help patients regain function, improve mobility, and maximize independence. You will work closely with physicians, caregivers, and interdisciplinary team members to ensure high-quality, patient-centered care.
Key Responsibilities
Patient Care & Treatment (60%)
Perform physical therapy evaluations and deliver treatments in accordance with physician orders.
Conduct OASIS assessments (SOC, Recert, ROC, Discharge).
Develop and implement individualized Plans of Care based on patient needs and goals.
Update treatment plans based on progress and clinical findings.
Provide patient and caregiver education on safe mobility, exercise plans, and self-care techniques.
Participate in Interdisciplinary Team Meetings to ensure continuity of care.
Communicate regularly with physicians, caregivers, and families on patient progress.
Complete home visits to implement therapy strategies that promote safety, independence, and improved function.
Assist with discharge planning and transition of care.
Team Leadership & Development (10%)
Provide guidance and clinical support to Licensed Physical Therapy Assistants (LPTAs).
Assist with staff onboarding, training, and performance feedback.
Support a positive, collaborative work environment.
Qualifications
Required
Active, unencumbered Physical Therapist (PT) license in the State of Ohio.
Experience working with geriatric populations.
Valid driver’s license, acceptable driving record, and insurance eligibility.
Ability to read, write, and communicate effectively in English.
Preferred
One year of experience in home health, hospital, or long-term care (new graduates welcome).
Strong knowledge of therapeutic techniques and treatment modalities.
