In fact, we’re the only hospital in northwest Ohio to receive this honor.POSITION SUMMARYThe Wound Care Licensed Practical Nurse provides and coordinates patient care, collecting patient history and wound information for the development of therapy plans under the direction of a provider or registered nurse. Educates both patients and their caregivers regarding the plan of care. Functions as case manager or team member as assigned for a specified group of patients.ACCOUNTABILITIES

Demonstrates knowledge, skills and exercises clinical competence in the clinic setting including but not limited to comprehensive wound evaluation, coordination of patient care, development and execution of therapy plans and all aspects of patient and family education.

Manages multiple priorities of working with a large caseload of complex wound patients in coordination with members of the health care team. Ongoingly reviews the therapy plan for patient response and effectiveness of treatment to promote optimal healing outcomes.

Collaborates with physician, patient/family and other health care team members in planning the patient’s care and treatment. Displays competence in giving directions in implementation of the patient’s therapy plan.

Remains current in practical knowledge of all regulatory and hospital guidelines, policies and procedures including safety education, hazardous waste management, emergency procedures, exposure control plan, adverse reaction reporting and fire/disaster planning.

Integrates knowledge from literature, research, education opportunities and networking into current standards of care and policy and procedures updates.

Seeks out and obtains advanced knowledge, credentials, specialty exams and training/education to enhance performance in the clinical role. Serves as a role model and preceptor.

Actively participates in implementing change to improve work environment, work processes or patient care while actively supporting and participating in Quality Improvement processes.

Assumes an active role in daily operations by attendance at staff meetings, in-services, and other Center related activities.

Travel and work assignments at system wound care locations is required. Travel mileage will be reimbursed from home (hired) location to alternate location.

Performs other duties as assigned.

REQUIRED QUALIFICATIONS

Education: Graduated from an approved school of practical nursing with a certificate, diploma or degree with medication administration course work.Skills:Ability to respond effectively and efficiently to critical situations involving patients, personnel and visitors in a professional manner.

Demonstrates sound clinical skills and judgment.

Ability to critique own daily practice for error and pursue corrective action.

Ability to deal effectively with a changing environment and be open to new ideas.

Functions as a dependable team member, assisting when requested and volunteering when the need is obvious.

Able to communicate effectively with all members of the health care team.

Demonstrates responsibility for continued profession growth, competence, and development.

Must be able to enter and retrieve information from a computer.Years of Experience: Must have a minimum of 2 years clinical experience; some ambulatory experience highly desired.   • Coordinates high quality patient care on assigned unit(s)/service. Provides staff support through facilitation, coaching, and role-modeling leadership behaviors, which enhance quality patient care, safety for staff/patients, excellent customer service, and fosters employee development.

  • POSITION SUMMARY Responsible for primary functions of a pharmacy technician: data entry, prescription fulfillment, packaging/shipping, customer service, and maintenance of pharmacy operations. Additionally, specialty pharmacy technicians perform outbound refill calls to patients, perform appropriate assessments, and triage to pharmacists if issues arise.

  • A Patient Care Tech (PCT), under the direction of licensed personnel, performs various patient care activities necessary in caring for a patient or a group of patients and performs clerical/receptionist services as required responsible for working a combination of nights, weekends, and holidays. The PCT will assist nursing/medical personnel in all areas while complying with established policies, procedures, and standards of practice in conjunction with all members of the healthcare team.
