Creative Options Service Coordinator Part-Time Position12/12/2024· Reports to the Muskingum County Family and Children First Council.· Office at the Muskingum County Adult & Child Protective Services on the East Pike Zanesville with the flexibility of working remotely when needed. A business cell phone will be provided.· This contract position is up to 20 hours per week with the flexibility of up to 25 hours per week due to increase in referrals or services needed.· Rate of Pay is $21.00 per hour. There are no benefits available. Taxes are your responsibility.· Education: Bachelor's degree in social work or related field. Must meet and maintain qualifications for driving on county business as a continued condition of employment. Service Coordination training and Wraparound Ohio Core training (or ability to obtain within 2 months of hire), CANS certified (or ability to obtain within 2 months of hire) knowledge of community resources, ability to work independently and with a team, computer skills and ability to learn OASIS system for Care Planning and documentation.· Job Responsibilities: The service coordinator will be responsible for providing service coordination for youth with complex, multi-system needs ages 0 through 21 and their families by implementing a wraparound services model, develop crisis stabilization plans, plans of care, completing CANS (Child Adolescent Needs and Strength Assessment), facilitate monthly meetings to determine progress, identify barriers and additional services needed, meetings with families to support the plans of care, facilitate meeting(s) including family and youth-serving partners in response to crisis or immediate needs. Make referrals to agencies and organizations for additional services. Facilitate the Creative Options Committee meetings. Update the Service Coordination Mechanism. Attend all meetings/trainings as required by Family and Children First Council. Communicate professionally and effectively with internal and external customers both orally and in writing. Responsible in assisting Family and Children First Coordinator in invoicing of bills, and data collection. The service coordinator will process MSY (Multi system Youth) applications as needed.
