Job Title: Registered NurseJob DescriptionWe are seeking a dedicated and compassionate Registered Nurse to join our team. The ideal candidate will be responsible for providing high-quality care to patients through telephonic triaging, coordinating follow-ups and referrals, and ensuring adherence to medical and departmental policies. This role involves working with complex cases and promoting cost-effective healthcare services.Responsibilities
Determine, develop, and implement a plan of care based on accurate and comprehensive assessment of the member’s needs through telephonic triaging.
Adhere to established medical/departmental policies, clinical practice guidelines, community resources, contracting, and community care standards to each case.
Work with complex cases, promoting the delivery of quality, cost-effective healthcare services based on medical necessity and contractual benefits.
Provide guidance to the provider network.
Set up appointments, follow-up appointments, track down labs, etc.
Coordinate follow-ups and referrals.
Handle 15-30 calls for telephonic information coordination each day.
Work with Medicaid patients in a team atmosphere.
