The Care Manager, Telephonic Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.The Care Manager, Telephonic Behavioral Health 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Bachelor's degree in SW or Behavioral Health related field
3 - 5 years of experience working in the field of SW or BH related field
1 or more years working with the pediatric population
1 year of managed care experience
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Preferred Qualifications
Master's degree in a behavioral-health field, such as social work, psychology, or related health discipline from an accredited university
License in SW or BH related field (LMHC, LMFT, LCSW)
CCM certification or must obtain within 1 year
Knowledge/Experience in navigation of community health and social service agencies and additional community resources
Experience in dealing with complaints and grievances related to members care and/or coordination of care.
Experience in participation in interagency meetings with community providers/members/families to address members care and tx goals
Key ResponsibilitiesThe Care Management of Pedatric member’s (Under 21) who utilize Applied Behavioral Analysis (ABA Services). Monitor/have awareness of utilization, conduct outreaches based on acuity to assess medical/social/BH needs and address gaps in case; participate in all scheduled Interagency planning meetings to discuss progress/treatment planning/d/c planning.
