This is a full-time field teleworker opportunity in Oklahoma. The US Military Veterans Wellness Specialist is the single point of contact to communicate with Veteran Service Organizations (VSO), the state Veterans Affairs, and Federal Veterans Affairs (VA) and Veterans services providers. Coordinates with VSOs, such as Veterans of Foreign Wars (VFW), American Legion, Disabled American Veterans, Homeless Stand downs for Veterans, each state has a Department of Veterans Affairs to coordinate state programs, Federal Veterans Affairs has numerous departments and systems that will need to be engaged and coordinate services such as; Homeless programs, Justice Outreach, Vet Centers, Community Based Outpatient Clinics (CBOC) and Suicide prevention programs. This position collaborates across multiple internal and external systems to support Veterans and their family’s needs.

Oversees the Veterans service delivery consistent with the state and federal programs and eligibility.

Identifies crisis response gaps and develops innovative solutions that improve care coordination with various private, state and federal resources.

Oversees the design and implementation of the integrated delivery programs [Behavioral Health (BH), physical Health (PH) and Social Determinants of Health (SDoH)] to ensure the needs of military veteran adults and their families.

Ensures collaboration with the justices’ system for justice involved veterans such as Veterans Treatment Courts and Veterans Justice Outreach.

Engages with the school systems on specific needs of military children

Coordinates with members of various State agencies to ensure that programs developed reflect the goals and values of the Veterans System Administrator.

Develops and implements practice adjustments and policy and procedure revisions.

Provides training and support internally and externally in effort to provide continuous improvement and increase knowledge of the intermingled systems.

Participates in shaping state integrated system of care and resolving issues and challenges identified.

Establishes and maintains on-going relationships with community organizations and contract stakeholders.

Ensures that the integrated (BH, PH and SDoH) care needs of members are well served.

Establishes and maintains ongoing relationships with key agencies, providers, physicians, hospitals, advocacy groups and State officials.

Creates effective organizational structure, roles and jobs.

Supports delivery of care coordination by participating in the Child and Family Teams, providing expert support to care coordinators, and collaborating with stakeholders such as state agencies and advocacy organizations.

Identifies and addresses the unique needs of our Veteran members and their families.

Required Qualification

Must reside in Oklahoma

Must possess lived experience as a US military Veteran or military family member

2+ years of experience in public or private sector, managed health care.

5+ years of experience working with Veterans and the VA services systems.

Ability to work beyond core business hours, Monday- Friday, 8 AM-5 PM.

2+ years’ experience using personal computer, keyboard navigation, navigating multiple systems and applications; and using MS Office Suite applications (Teams, Outlook, Word, Excel, etc.)

Must possess reliable transportation and be willing and able to travel in-state up to 25% of the time. Mileage is reimbursed per our company expense reimbursement policy.

Pay RangeThe typical pay range for this role is:$54,300.00 - $119,300.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
