One of Central Oregon's premiere employers is seeking an experienced Family Nurse Practitioner to join our hospice team in Central Oregon.

It is the oldest and largest single hospice provider--including one of the state's three in-patient hospice facilities--in a 10,000 square mile region.

JOB SUMMARY

Responsible for reviewing the status of the hospice patient with the IDT prior to conducting the required face-to-face visit as designated by the Hospice Medical Director; conducts a thorough comprehensive assessment of the patient in order to support evidence of decline and a continued need for hospice if appropriate; completes appropriate documentation with 48 hours of visit; timely execution of e-scribe orders; provides education to clinical and other staff; mentors and serves as a clinical resource for hospice field RNs; collaborates as part of a team of physicians and nurse practitioners with regular meetings; responsible for efficiently coordinating and scheduling face to face visits considering geography/travel time and patient complexity; makes patient visits in the field for those requiring more intensive symptom management as requested by hospice team; reviews medications and recommends changes appropriate to individual cases; communicates findings to Medical Director and the rest of the IDT involved in case. attends twice weekly IDT meetings; serves as PCP for patients when applicable; travels throughout Central Oregon service area including LaPine, Crescent, Christmas Valley, Prineville, Madras and Sisters.

Take call M-F from 8AM to 6PM

POSITION QUALIFICATIONS

Educational requirements include graduate of an accredited school of nursing with current licensure to practice medicine in the State of Oregon. Two or more years of previous experience preferred in hospice. Valid Oregon drivers license and/or reliable transportation with current auto insurance as applicable.
