We’re a trusted provider of in-home healthcare services throughout the Eugene/Springfield area, known for our client-centered approach and strong team culture. As we grow, we’re looking for an RN who values autonomy, collaboration, and making a genuine difference every day.
Key Responsibilities
Conduct in-depth intake assessments and craft individualized care plans
Perform quarterly reassessments and update care plans as needed
Delegate tasks appropriately to care team members
Monitor progress and adapt care plans to meet changing needs
Educate clients and their families on conditions, treatments, and self-care
Develop caregiver training materials to satisfy the annual caregiver continuing education
Coordinate care closely with the clinical administrator, caregivers, and other providers
Maintain accurate, timely documentation
Communicate regularly with office administrators to support operational needs
Qualifications
Active Oregon RN license
2+ years of clinical nursing experience (home health experience preferred)
Strong critical thinking and clinical judgment
Compassionate, clear communicator – with clients and teammates alike
Ability to work independently with integrity and professionalism
