Community Health Worker
Remote to start but candidate must be located around the following areas: Lehigh Cap and Pittsburgh
Field Travel Needed over 50% of the time (Travel will be reimbursable)
Contract through the end of year with possible contract to hire (conversion rates ($20-21)
Work schedule Monday-Friday with the weekend work once a quarter
The Community Health Worker (CHW) will be responsible for helping members to navigate and access medical care, community services/resources and develop healthy behaviors. The CHW will act as a caring neighbor to help members address the social, medical and community problems that lead to poor health (SDoH/HRSN). As a priority, interactions will promote, maintain, and improve the health of members and their families. This includes to provide social support and advocate for individuals and community health needs.
Establish trusting relationship with members.
Make follow up calls and community visits to ensure member is connected to community services.
Meet member in medical setting to ensure member has comprehensive and coordinated care.
Assist member with completion of applications for medical or social benefits.
Document all member encounters in documentation platform.
Maintain excellent working relationships with members, families, providers and CBOs.
Knowledge of community resources
Qualifications:
High School Diploma or GED completed.
Bilingual – English and Spanish
Lived experience in the community served.
Ability to become a certified CHW.
