Position Title: Nurse - Navigator Program

Summary of Responsibilities:

Under the general direction of the Director of Operations for Franklin/Fulton Counties, the Nurse Supervisor is responsible for working with high-risk individuals to increase the satisfaction of current health participation, increased attendance to medical appointments and reduce hospitalizations. Responsibilities include face-to-face contacts with individuals twice per month, community outreach, and weekly collaboration with medical professionals. This Supervisor will also provide weekly supervision, training, and debriefing with Case Managers. Provides daily administrative management of the SAM Inc. Franklin/Fulton Nurse Navigator Program.

This individual will have a thorough understanding of internal operations and the operational relationship SAM, Inc. has with contracted providers, payers, as well as Local and State Government entities. Operations are conducted in accord with all existing policies, procedures, and regulations and in accord with the Mission and Values of the Company.

Essential Functions:

Complete and review assessment tool for all physical health concerns

Train and educate individuals on personalized physical health concerns

Attend appointments with the PCP as requested/needed

Review lab results/ other physical health assessment information and completes education based on the results

Complete health assessments and wellness checks

Facilitate community training as needed and support groups as needed/identified by Individuals

Attend meetings as needed/requested by Individuals

Provide community outreach to discuss and promote the program

Review individualized wellness goals and provide updates/education on specific health/wellness concerns

Provide consultation during emergencies

Complete reports as needed

Conduct research, training, and educational meetings

Provide supervision, training, and guidance for Intensive Case Managers on a daily, weekly basis as needed basis to ensure adherence to program, payer and regulatory requirements

Conduct an on-going review of the service documentation, including case notes for persons receiving ICM Services

Review individual service recipient records to determine the appropriateness and timeliness of service plans and case management interventions

Prepare reports reflective of necessary Program/ICM data elements/ program outcomes as needed

Maintain cooperative, collaborative and professional relationships with service providers, community agencies and institutions

Other Functions:

Other duties as assigned by the Director of Operations, Franklin/Fulton County

Supervisory Responsibilities:

1-2 Intensive Case Managers

Minimum Requirements: (Education, Experience, etc.):

BSN

Registered Nurse

At least 3 years of mental health direct care experience

Required Licenses, Certificates or Knowledge:

Excellent computer skills; proficient in Excel, Word, Outlook

Excellent communication skills; both verbal and written

Excellent interpersonal skills and a collaborative management style

A demonstrated commitment to high professional ethical standards and a diverse workplace

Open to direction and feedback

First Aid/CPR

HIPPA

Minimum Employment Requirements:

Automobile

Valid Pennsylvania Drivers License and acceptable MVR

Act 33 and Act 34 Clearances

Social Security Card

Immigration and Naturalization (I-9) Clearance

Drug screening clearance

FBI Clearance

Working Conditions:

Working out of law enforcement or provider offices, and in the community at large

Safety Hazards of the Job:

Some travel may be during adverse weather conditions

Will have contact with Individuals who may have a mental health disturbance, are suicidal, suicide in progress or an emergency petition
