Under the direction of the Director of Nursing, the successful candidate will work within their entire scope of practice. The LPN will be fully accountable for ensuring that the utmost in competent care and safety is consistently delivered to all patients. This opportunity awaits a motivated and ambitious professional with assisted living experience.Key Functions:
Provides direct, individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Nursing and/or a Registered Nurse designee.
Ensures quality and safe delivery of home health services to patients/clients and families/caregivers.
Implements plan of care formulated by physician and Registered Nurse.
Provides accurate and timely documentation consistent with plan of care.
Assesses and provides patient/client and family caregiver education and information pertinent to diagnosis and plan of care.
Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards.
Participates in coordination of home health services with Registered Nurse & clinical management.
Assists patient in learning appropriate self-care activities
Uses equipment and supplies effectively and efficiently.
Documents and reports activity as required.
Other Licensed Practical Nurse (LPN) duties as assigned.
Qualifications:
Must be at least 18 years of age
Licensed in the State of PA & qualified to pass medications
One year of clinical experience or other appropriate experience
Certification in CPR for health professionals
Valid driver's license with access to a reliable vehicle, proof of insurance & of registration
Current PPD or Chest X-Ray
Eligibility for State Police Criminal Record Check & Child Abuse History Clearance
FBI clearance required
Ability to work independently and manage time effectively.
Ability to travel to cases as assigned
