The Licensed Practical Nurse (LPN) functions collaboratively within the team by observing the patient/family needs and facilitating care. The LPN provides direct nursing care to home health or hospice patients within the scope of practice for LPNs. The individual will follow the Plan of Care (POC) established by the RN and the Interdisciplinary Team for patients.

Responsibilities:

Provides home health or hospice services to patients through regularly scheduled home visits.

Follows established Plan of Care (POC) developed by the RN and understand the process for changing the POC according to the patient and family needs.

Performs approved nursing procedures and treatments within the LPN scope of practice. Follows appropriate preventative and palliative nursing procedures according to the Nurse Practice Act for LPNs.

Documents all interventions, patient/family response, and medication administered in the medical record. Maintains patient records so that problems, plans, actions, and goals are accurately and clearly stated.

Completes patient notes the same day that patient is seen or shift care provided.

Assists in the planning and evaluation of the nursing care for patients and their families.

Reviews POC with the RN and RN Team Manager. Updates and revises the POC under the direction of the RN to address current patient and family needs.

Participates in performance improvement activities as requested.

Credentials:

Basic Cardiac Life Support (Required)

BLS/CPR, as a healthcare provider as per the American Heart Association required.

Car Insurance (Required)

Driver's License (Required)

Valid Driver’s License requiredReliable personal vehicle required

Licensed Practical Nurse (Required)

Pennsylvania and/or New Jersey LPN licensure depending on geographic location and scope of responsibilities required.

Education or Equivalent Experience:

H.S. Diploma/GED (Required)

Education Specialization:Graduate of an NLN accredited licensed practical nursing program

Equivalent Experience:2+ years Acute care, home care, hospice care, or long-term care experienceFull time experience as a Licensed Practical Nurse

  • Summary This position is located in Region I, Division of Radiological Safety & Security, Decommissioning, ISFSI & Reactor Health Physics Branch. The supervisor is Anthony Dimitriadis, Branch Chief. This position is subject to Confidential Financial Disclosure and Security Ownership Restriction reporting requirements. Responsibilities The successful candidate will serve as a senior inspector to lead, plan, conduct, report and integrate violations and findings associated with radiation protection inspection of Independent Spent Fuel Storage Installations (ISFSI), operating light water reactor facilities, inspections of decommissioning reactors and/or materials facilities, in accordance with the NRC inspection manual. You may also perform and coordinate radiological and environmental protection licensing of varied facilities using byproduct, source and/or special nuclear materials. Investigates and evaluates the most difficult occurrences with radiological safety significance. Such duties include but are not limited to: 1. Performs a wide variety of lead duties associated with planning, conducting and leading challenging or routine inspections of ISFSI programs and radiation protection programs at operating reactor facilities and decommissioning reactor facilities. 2. Prioritizes safety and compliance issues, and initiates enforcement action when inspection findings indicate a significant violation of the NRC rules and regulations or license conditions. 3. Prepares individual reports and coordinates team reports of inspection findings to document whether the facility is being operated safely and/or in accordance with the requirements. 4. Performs miscellaneous technical and administrative duties in support of the materials licensing and regional inspection programs. 5. Serves as a mentor and coach in the development of new inspectors, providing guidance and facilitating knowledge transfer of critical knowledge and skills to new employees and inspectors undergoing the qualification process. Qualification under any appendix in Inspection Manual Chapter (IMC) 1245, 1246 or 1248 is required. Additional qualification under IMC 1246 App B3 is preferred.
