Located in the heart of scenic, rural Western Pennsylvania, Armstrong County Community Action Agency, Inc., d/b/a Armstrong County Community Action Partnership (ACCAP),is a 501(c)(3) nonprofit agency incorporated in 1965. We are a community-based, grassroots organization serving individuals facing economic hardships. Over the years, ACCAP has established itself as one of the single largest providers of comprehensive human services within Armstrong County.Our agency is seeking a talented individual to join our Housing Placement team as a full-time Housing Case Manager. The following is an outline of the necessary tasks and duties
performed by the Housing Case Manager:DUTIES AND RESPONSIBILITIES:
Responsible for developing an Individualized Service Plan (ISP) with the goal of assisting agency clients in obtaining the necessary supportive services to promote self-sufficiency.
Submit ISP plan for review and approval to direct supervisor.
Periodically review ISP to insure goals are being addressed and met.
Maintain regular and consistent communications with clients/potential clients in a time and professional manner including initiating home visits as needed.
Write compressive and accurate case notes in a consistent and timely manner.
Maintain accurate client files and records according to specified funding sources, department regulations, and assigned caseload.
Ensure delivery of services in a timely manner.
Provide budget and family finance management counseling.
Attend, and participate in, regular case management meetings with the Housing Placement Manger.
Coordinate activities relating to clients with other social service agencies.
Ensure client tracking activities are properly carried out as required by the Housing Placement Manager in order to assess the self-sufficiency of the programfollowing the client's release from the program.
Complete direct client services to the homeless, near homeless, or low-income residents involved in any housing or utility assistance, as well as any emergency food and shelter programs on behalf of the agency.
Complete and enter client data correctly into various database platforms.
Complete transmittals and vouchers pertaining to client services.
Network with the appropriate social service agencies, utility companies, landlords, real estate agencies, or any other groups relating to client assistance in order to enhance coordination efforts and the delivery of services to agency clients.
Keep comprehensive and accurate records of client statistics and assist in preparation of all appropriate reports, including quarterly and year-end reports, for submission to the respective funding sources. All documents must be submitted to the Housing Placement Manager for review and approval prior to submission.
Participate in the Homeless/Housing Advisory Committee meetings to share information about homeless, near homeless, and low income clients, as required,in the yearly program evaluation process.
Attend meetings as required by the Executive Director, Housing Placement Manager, or his/her designees.
Search and locate rental apartments for clients using all available resources including phone calls, emails, internet, newspapers, etc.
Perform rental inspections and coordinate environmental reviews with the county contact.
Coordinate with clients and landlord in obtaining signatures in leases.
Assist with other Housing Department tasks as needed.
Travel to on-site locations appropriate to duties and responsibilities when necessary, and as instructed by the Executive Director, Housing Placemat Manager, or his/her designees.
Abide by the agency policy that contacting state or federal funding sources is prohibited unless instructed by the Executive Director, Housing Placement Manager, or his/her designees.
Complete any and all duties assigned by the Executive Director, Hous ng Placement Manager or his/her designees.
The successful candidate will possess the following qualifications:
Must have a minimum of a high school diploma, or its equivalent (GED); Associates Degree or post-secondary education preferred (but not required) in psychology, sociology, counseling, education, communications, or related discipline.
A minimum of one year related experience and program management experienced preferred, but not necessary.
Computer literate and possess aptitude for learning new software programs
Effective communications skills.
Possess excellent organizational and problem-solving skills.
Ability to ensure the successful creation and completion of departmental goals.
Identify and resolve problems and accomplish goals.
Demonstrate the ability to work independently with little or no supervision.
Demonstrate the ability to work as a team.
Familiarity with Microsoft Office and Excel helpful.
Able to operate basic office equipment such as telephone, copy and fax machines, and postage machine.
Ability to successfully coordinate with County and State agencies.
Must be empathetic with regards to the needs of the low-income and economically disadvantaged.
Must be able to lift 20 pounds.
Must be able to sit at desk to complete reports, attend Zoom trainings, etc.
Other duties as assigned.
PA State Criminal Background Check, PA Child Abuse History Clearance, FBI Fingerprint Criminal Records Check, and Drug Testing Required.
Valid driver's license and good driving record required.
Personal vehicle or access to independent transportation required.
Proof of motor vehicle insurance (for use of personal vehicle).
Must adhere to agency confidentially standards at all time.
Social services experienced preferred, but not necessary.
Professional appearance; positive attitude.
Adhere to the agency's Confidentiality Policy.
