OverviewThe Care Coordinator supports children and families through their treatment journey with CHI St. Joseph Children’s Health by facilitating the collaborative care model within the CHI St. Joseph Children’s Health family-focused behavioral health model. The Coordinator maintains open communication with appropriate external medical providers with whom the organization may be collaborating and coordinates internal behavioral health services and educational services for the child/family enrolled in behavioral health services as a part of their treatment plan.Support services provided by the Care Coordinator focus upon the development of and completion of treatment strategies for children and families and includes:
Developing and maintaining the service network necessary to ensure that children and families receive appropriate care,
Ensuring that that patients and families have access to and engage with required and recommended programs
Coordinating behavioral health care plans internally and externally to ensure coordinated and collaboratively work to achieve the best outcomes for both the patient and their family.

Coordinate the behavioral health program enrollment process for children and families
Schedule and facilitate the patient on-boarding process to the care model
Monitor patient and provider compliance with agreed upon treatment plans and activities as all elements of the program are engaged and delivering services to the child/family.
Ensure that care plans are coordinated and collaborate to ensure the best health outcomes for enrolled children and families.
Link patients and families to appropriate internal and external medical services essential to fulfilling their treatment plans and maintaining care.
Document all activity with patients and families. Provide reports on activities and outcomes of services provided.
Maintains compliance with regard to state, federal and CHI guidelines.
QualificationsEducation (including licenses, certifications):
Bachelor’s Degree in Social Work, Sociology, Psychology or other related field required
Experience required and preferred:
Minimum of 1 years experience in working with families in a casework/case management capacity or medical care coordination capacity.
Bi-Lingual (Spanish) strongly preferred.
Experience with setting and tracking outcomes in a human service field.
Skill Competencies:
Excellent communication skills – both written and verbal
Strong facilitation skills
Ability to handle multiple tasks
Comfort working with computers and with utilizing tele-psych technology
Demonstrate a high comfort working with diverse populations and an understanding of the cultural and social barriers that can exist when accessing health or health-related care.
Promote a positive team-based approach to treatment and care.
