Case Manager/ Administrative Assistant (Entry Level) - Full Time (with some weekends)

This position provides counseling and casework services to reentrants/parolees with behavioral and adjustment issues in our Community Correctional Facility.

20% Interviews parolees to facilitate their institutional adjustment, assists with issues that are inherent in a community correctional setting, and/or provide encouragement. Provides consultation and assistance in resolving personal and family problems, conflict resolution, and dealing appropriately with other concerns that may be encountered within the facility as well as in the community. Provides referrals and guidance for reentrants to develop social, educational, and vocational skills that are necessary to make a positive re-entry into the community. Aids in obtaining information regarding employment opportunities, procedures for job applications and retention, and availability of community resources. Assist in obtaining Medical and Food assistance services for them as needed. Starts or completes SSI applications for those who qualify.

20% Develops and maintains necessary paperwork, reports, RFP’s etc for managing your caseload. Fosters positive relationships between the facility, other departments, and community agencies by providing necessary information for effective case management and community reintegration. Confers with Director, Lead Case Manager & RD about a parolee’s adjustment, including work supervisor, treatment attendance and other Team members.

20% Utilizes assessment scores to determine recommended programming and risk level, compiles documents to include socio-demographic histories, and maintains documentation regarding sentencing, employment, medical, vehicles, psychological/DA evaluations, assessment scores, correctional planning, etc.

20% Utilizes a multi-disciplinary approach to coordinate treatment objectives to include: programming, activities, counseling, and educational services in a manner that supports effective utilization of personnel and material resources. Facilitates compliance and behavioral change through the implementation of Infractions, Behavior Contracts and Disciplinary conferences inclusive to expectations regarding work and housing, overall behavior, academic and vocational programming, and recommended programming based on need and assessments. Meets with Team for job changes, program/custody level changes, parole recommendations/conferences, informal misconduct resolutions, etc.

15% Participates in parole planning and makes recommendations regarding re-entry services. Secures forms of identification and assists with community reintegration. Sets up counseling appointment dates and ensure attendance. Assist with SSI applications and follow ups.

5% Performs other related duties as assigned.

ESSENTIAL FUNCTIONS:

1. Effectively communicate verbally and in writing.

2. Conducts New Intake paperwork; Orientations for all new parolees.

3. Enters Daily/Weekly Case Notes into Documents and in shared server.

4. Maintain and update all weekly logs (work, treatment, appointments, etc.) for your caseload.

5. Word, Excel and Outlook proficiency is a MUST.

6. Experience with online applications for Food Stamps, Medical assistance, SSI applications etc. is a PLUS.

7. Calls to ensure parolee employment and treatment compliance.

8. Works with community liaison for services as needed.

9. Review, analyze, and interpret information from Parole and DOC.

10. Maintain regular and reliable attendance.

11. Operate basic office equipment – Copier, Scanner, etc.

12. Establish and maintain effective working relationships.

13. Attend and participate in all mandatory training.

14. Visually observe parolee activities.

Will need to work 2 Saturdays in a month.
