As a Case Manager you will:
Coordinate care of individuals in mental health, medical, educational, placement and related areas. Assess needs using identified tools to develop initial plan of care
Connect families to needed resources in their local community - which could include educational, financial, mental health, physical health, housing, food insecurity, and other resources.
Coordinate monitor and review the provision of services to ensure individuals are receiving and participating in services on a continual basis.
Maintain direct contact (telephone and/or face to face) with all individuals involved in assigned caseload per state regulations.
Ensure the timely and accurate completion of related documentation to include service documentation, outcome reports, service plans, face sheets, FPI, authorizations, releases and assessments.
The home office is located in Malvern, PA however this position primarily works in the community and visiting the homes of assigned clients- mostly in the Chester County area. Vehicles are provided for work travel.Devereux Pennsylvania Children’s Services (DPACS) serves children, adolescents and young adults with autism spectrum disorders, intellectual and developmental disabilities, behavioral, emotional and psychiatric challenges and disorders. Located in West Chester and Malvern Pennsylvania, services provided include psychiatric residential treatment (PRTF), acute inpatient psychiatric hospital (AIP), assessment services, community-based mental health services, community-based employment training and social skills training, and approved private school educational services to male and female children and adolescents (6-21 years of age) with diagnoses of intellectual disabilities, autism spectrum disorders, concomitant psychiatric and behavioral challenges often associated with experiencing some level of trauma in their lives.#sponsoredQualificationsEDUCATION: Associate’s degree with 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years’ experience in public or private human services with 1 year in direct client contact. Or Bachelor's Degree in Human Services field (Psych, Social Work, Sociology, Criminal Justice, etc)E XPERIENCE: 2 years’ experience in public or private human services with 1 year in direct client contact.Devereux has a zero-tolerance policy for abuse and maintains policies and procedures to systematically reduce the risk of abuse to occur in our organization.
