In this position, you will promote, protect, and improve a continuum of care designed to help older adults sustain independent, meaningful, and dignified lives in their homes and communities for as long as possible. You will also be the main point of contact for cases involving abuse, exploitation, abandonment, or self-neglect, ensuring adherence to protocols by preparing a thorough Report of Need. Your responsibilities will include providing information and referral services, aiding clients in applying for various benefits such as PACE/PACE Net, Medicare/Medicare Part D, Property Tax & Rent Rebates, and LIHEAP. Additionally, you will inform consumers about available services, programs, and community resources, and refer them to other agencies or service providers when needed. Moreover, you may serve as the PA-MEDI Coordinator and deliver Person Centered Counseling to clients.Work Schedule and Additional Information:Full-time employment
Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:One year as an Aging Case Aide 1; or
Two years of public contact work, including one year in a human services program; or

Hiring preference for this vacancy may be given to candidates who live within Carbon, Lehigh, Luzerne, Monroe or Schuylkill counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
