Description/Job Summary/Direct Support Professional/Position SummaryThe Direct Support Professional for RHD's Co Star advocates for and advises people in recovery (PIR) in their home and in the community; while teaching and assisting with skills building in relationship to residential, educational, vocational, social, recreational, spiritual, and self-maintenance domains, by performing the following duties.Essential Duties and Functions

Assists Individuals with maintaining households, such as changing bed linens, washing and ironing, laundry, and cleaning quarters.

Provides individuals with assistance while purchasing, preparing, and serving nutritious foods.

Provides assistance in following special or prescribed diets.

Provides assistance dressing, bathing, and grooming self.

Supervises the self-administration of prescribed oral medications under written prescription of Physician.

Supports individuals with traveling outside of the home.

Assists individuals with finding and involving self in daily activities that provide meaning to his/her life

Explains fundamental recovery principles and renders bedside care to individuals who are ill.

Participates in evaluating needs of Program participants, and confers with family members, case managers, and others in Program participant s support network to plan for continuing and additional services.

Performs variety of miscellaneous duties as requested, such as assisting Program participants in obtaining household supplies and other needs.

Maintains records of services performed and of apparent condition of PIR.

QualificationsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily as well as possess the ability to input daily activities onto computerized documentation. High school diploma or general education degree (GED).

At least one to two years of experience working in a MH residential setting

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to read routine reports and correspondence.

Ability to speak effectively before groups of participants or employees of organization.

All employees must possess and maintain a current driver s license

Ability to add, subtracts, multiply, and divides in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Program SummaryCOSTAR is a community living residential program forindividuals with co-occurring mental health and/or intellectual disabilitychallenges. COSTAR individuals are often older, require slight medicalattention or special care and live in two to three-person sites with staffsupport nearby. Because individuals are often older, participants do not“transition or “graduate to a lower level of care or independent living.Staff, including Program Coordinators, work with individuals to strive to reachtheir recovery goals and ingrate into the community. Staff utilize astrength-based, person-centered approach to encourage individuals to attendmeaningful activities in the community at organizations that COSTAR haspartnered with. For example, local community centers often host evidence-basedgroups like the Trauma Recovery Empowerment Model (TREM), trauma recoverygroups, or Wellness Recovery Action Planning (WRAP) workshops at local recoverycenters. COSTAR s goal is to address thespecial needs of the individual, build daily living skills, connect to behavioralhealth resources and wrap natural and formal supports around them to aid inrecovery.
