As a Registered Nurse Assessment Coordinator, you will play a crucial role in ensuring the well-being of residents at the veterans' home. You will coordinate and oversee the collaborative, interdisciplinary assessment, and care planning process, which includes conducting comprehensive resident assessments, coordinating and planning care, advocating for residents, and facilitating communication among team members, residents, and families. In addition, you will be involved with quality assurance activities, external site visits for the clinical review of residency applications, and staff training programs to enhance the quality of resident care.Interested in learning more? Work Schedule and Additional Information:
Full-time employment (40-hour work week)
Work hours are 1st shift (6:00 AM to 2:30 PM), rotating weekends and holidays, with a 30-minute lunch.
Salary:In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITYQUALIFICATIONS
Minimum Experience and Training Requirements:
One year of clinical nursing experience in a long-term care facility including the performance of MDS-based nursing assessments and the development of individualized nursing care plans; orPossession of an active certification as a Resident Assessment Coordinator - Certified (RAC-CT) as issued by the American Association of Nurse Assessment Coordination (AANAC).Special Requirements:This position requires active authorization to practice as a Registered Nurse in Pennsylvania.All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.Other Requirements:
PA residency requirementis currently waived for this title.

A conditional offer of employment will require a medical examination.
This position falls under the provisions of the Older Adult Protective Services Act.
