Key Responsibilities:
- Provide comprehensive social services to residents, including psychosocial assessments, individual and group counseling, and support with adjustments to aging and health changes.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Advocate for residents' rights and ensure their needs are met through coordination with families, medical professionals, and community resources.
- Facilitate communication and conflict resolution between residents, families, and staff.
- Plan and conduct educational programs for residents, families, and staff on aging, mental health, and community resources.
- Assist with discharge planning and transition of care as needed.
- Maintain accurate and confidential records of all resident interactions and interventions.
Qualifications:
- Master's degree in Social Work (MSW) preferred from an accredited institution.
- Minimum of 5 years of experience in a social work role, with at least three years in a senior living or long-term care setting.
- Strong understanding of the physical, emotional, and social aspects of aging.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of an interdisciplinary team.
- Empathy, patience, and a genuine commitment to improving the quality of life for seniors.
