Follow the Plan of Care to assist participants in maintaining acceptable hygiene and a safe environment and performing only those functions specified for each individual participant.
Reports any unsafe or unusual circumstances or changes observed in participant's health or physical condition to immediate supervisor, including but not limited to, critical incidents, falls, hospitalizations, etc.
Ability to display an empathetic attitude while caring for the participant.
Receives written and verbal instructions from the supervisors and receives performance feedback and instructions, calmly without reacting negatively or emotionally.
Is oriented and trained in all aspects of care to be provided to participants, and can demonstrate competency in all areas of training, including but not limited to the following:
1. Medication reminders, assistance with delivery of non-narcotic medications.
Housekeeping, including but not limited to, washing dishes, cleaning restroom facilities, vacuuming, dusting, taking trash out, and mopping.
Personal care, including bathing, dressing, grooming, hair care, and shaving.
Meal preparation, assisting with setup, feeding assistance, and kitchen clean-up.
Oral hygiene and denture care, twice a day.
Toileting, post-toilet hygiene, cleaning urinals, bedside commodes, bedpans, etc.
IADL assistance
Administering basic first aid
Providing or arranging for social interactions within the community.
Providing or arranging for transportation, as necessary
Arranging and reminding participants of medical appointments and therapies
95% Compliance with Electronic Visit Verification (EVV).
Performs other duties as assigned.
Required Qualifications:
Ability to read, write, communicate and verbally communicate and follow directions is essential.
Demonstrated competency by passing a competency exam developed by the home care agency which meets PA state regulation.
Basic understanding of bathing, grooming, light housekeeping, medication reminders, and meal preparation.
Effective and timely communication with all staff and participants.
Understanding and respecting participants, including but not limited to ethical standards and confidentiality of care, such as adhering to HIPAA regulations.
Must demonstrate reliability, dependability, and trustworthiness.
Accountability to establish professional and respectful interpersonal relationships.
Must undergo a PA Criminal Background Check and other required checks as required by PA Department of Health, OLTL, and Department of Aging regulations and polices.
Must not be excluded from participating in Medicare, Medicaid, or any other federal health care program.
Completion of mandatory annual training each year.
CPR and First Aide certification is optional.
Multilingual abilities are preferred.
