The Center for Liver Care is seeking an intake coordinator to join our team, where they will play a vital role in patient access, scheduling office visits, and coordinating procedures. The ideal candidate will be highly organized and detail-oriented, ensuring accuracy and effective follow-up. This full-time position is based in the Kaufmann Medical Building and operates Monday through Friday during daylight hours.Responsibilities:
Manage initial intakes of referred patients referred to the UPMC Center for Liver Care (CLiC).
Asist with the coordination of care for patients with liver disease, in collaboration with the medical team.
Manage referral source missing information.
Participate in multidisciplinary care of patients with liver disease with nurses, advances practice providers, physicians, and other medical personnel.
Obtain basic medical histories and personal information of new patients.
Update medical records of new and existing patients.
Review documents for accuracy and completeness.
Maintain accurate documentation.
Enter data into the electronic medical record.
Maintain weekly/monthly productivity.
Collects initial referral information via telephone, fax, or e-mail and distributes information to appropriate team member in a timely and efficient manner.
Demonstrate the ability to establish priorities, perform assignments and meet productivity thresholds in the allotted time frames while maintaining accuracy and attention to detail.
Support Transplant Coordinators and other members of the Transplant Team.
Maintain patient confidentiality.
Coordinates and manages living donor evaluation including but not limited to scheduling all required appointments and testing, obtaining authorizations, staffing donor clinic and preparation for donor selection committee meeting.
General data entry responsibilities to include the collection and verification of demographics and insurance information, test results, and other pertinent information into and EPIC databases.
Prepare, organize and maintain confidential patient charts and clinical records.
High school diploma or equivalent required.
Associate degree preferred.
Two years' experience with administrative support in an outpatient clinic, physician office or other health care setting.
Knowledge of medical terminology required.
Ability to handle a high-volume workload and to remain focused and organized under pressure.
Ability to multi-task and meet deadlines.
Ability to perform duties with significant degree of independence and initiative.
Strong interpersonal, communication, and customer service skills required.
