A Personal Care Assistant is an individual who providessupport services to assist clients with personal, physical mobility andtherapeutic care needs, as established by a rehabilitation health practitioner,social worker or other health care professional. The Personal Care Assistantmay work in a variety of settings, to include medical facilities,rehabilitation centers or in-home residential care. Essential Duties and Responsibilities:
Provides services to persons who need help with day-to-dayactivities
Demonstrates competence in performing the necessary skillsbased on the patient’s plan of care
Provides health care tasks, personal hygiene services, andother related support services essential to the client’s health and safety
Assists with transfers and ambulation
Performs incidental household services that are an integralpart of a personal care plan (if applicable)
Provides companionship by reading, listening and talkingwith client
Assists client in the development of independent skillsthrough self-care activities such as toileting, clothing routines, and personalhygiene
Implements behavior management techniques in accordance withthe directions given by the supervisor/healthcare practitioner; includingreinforcing appropriate behaviors in accordance with plans and discouragingnegative behavior
Reports changes in patient/client status
Adheres to the Health Insurance Portability andAccountability Act (HIPPA) privacy policies and procedures
Performs other duties as assigned/necessary
Minimum Requirements:
State licensure or certification if required by state and/orcontract
One (1) year relevant patient/client care experiencepreferred
Complies with all relevant professional standards ofpractice
Participation and completion of Amergis' Competency programwhen applicable
Current CPR if applicable
TB Questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language may be required
Computer proficiency required
Must be at least 18 years of age
